permission letter

do any of you know what needs to be included in a letter of permission to cover yourself or even better put a draft up for me to copy being the imagineitive person i am harddigging.

thanks for that its appreciated i thought there would of been some complecated lingo to be involved some where but i think even i can manage to print some of them out to be signed thank you very much harddigging
